This post will give a little bit in long descriptions about the components working in HIV/AIDS programs in a state which are situated in different health sectors for promoting testing increase and controlling HIV infections under the State Aids Control Society.

Different components in HIV/AIDS Program

Most of these are in government hospitals, which offer STI, ICTC, PPTCT, ART CENTERS, BLOOD BANK.

Integrated Counselling and Testing Centre(ICTC)

There are 20,756 Integrated Counselling and Testing Centres (ICTC), mainly located in government hospitals.

An ICTC is a place where a person is counselled and tested for HIV, of his own consent or as advised by a Doctor.

The main functions of an ICTC are:-

Conducting HIV diagnostic tests, Providing basic information on the modes of HIV transmission, and promoting behavioural change to reduce vulnerability. Link people with other HIV prevention, care and treatment services. Ideally, a health facility should have one Integrated Counselling and Testing centre for anyone. However, an ICTC is located in facilities that serve specific categories such as high-risk groups, pregnant women, STI cases, TB Patients, HIV/ AIDS symptomatic patients. Accordingly, an ICTC is located in the General OPD or Obstetrics and Gynaecology Department of a medical college or a district hospital or in a maternity home where the majority of clients can access counselling and testing services.

Prevention of Parent to Child Transmission (PPTCT)  

Most of these in government hospitals, which offer PPTCT services to pregnant women. to prevent the perinatal transmission of HIV from an HIV infected pregnant mother to her newborn baby.  pregnant women who are found to be HIV positive are initiated on lifelong ART irrespective of CD4 count and WHO clinical Staging; their new-born (HIV exposed) babies are initiated on 6 weeks of Syrup Nevirapine immediately after birth so as to prevent transmission of HIV from the mother to child and is extended to 12 weeks of Syrup Nevirapine if the duration of the ART of mother is less than 24 weeks.

The HIV exposed baby is initiated on Cotrimoxazole prophylaxis at 6 weeks and is tested for HIV DNA PCR at 6 weeks by DBS (Dry Blood Spot) collection. If the DBS sample is positive for HIV DNA PCR, then a repeat DBS sample is tested for HIV DNA PCR. The HIV exposed baby is then initiated on lifelong ART at the earliest if confirmed HIV positive through 2 DNA PCR tests. HIV counselling and testing service is a key entry point to the prevention of HIV infection and to treatment and care of people who are infected with HIV. 

When availing of counselling and testing services, people can access accurate information about HIV prevention and care and undergo HIV tests in a supportive and confidential environment. People who are found HIV negative are supported with information and counselling to reduce risks and remain HIV negative. People who are found HIV positive are provided psycho-social support and linked to treatment and care.
 
HIV/Tuberculosis collaborative activities:-

The Basic Services Division through Integrated Counselling and Testing Centres (ICTCs) provide HIV counselling & testing services, Prevention of Parent to Child Transmission of HIV (PPTCT) services and HIV-TB cross-referrals. These services have linkages with TI NGOs/CBOs and outward linkages with ART centres, outreach services, peer support services, OI management and home-based care.

STI/RTI CLINIC

STI/RTI Clinics are situated at the District level in Government District Hospitals or where PMO(Principal Medical Officer) is sitting in a Government Hospitals. NGOs are working on High-risk Populations which are residing's, there STI Clinics is Created by the State Aids Control Society so that High-risk groups of peoples should be identified and promoted for testing for HIV and syphilis.

In STI clinic usually, counselling and kits are given to the patient who visits the clinic with symptoms or not thereafter counselling they are promoted to go for physical examination by the doctor and referred to General Lab for SYPHILIS(RPR Test) and to ICTC for HIV test and then explaining why testing is necessary after collecting the report than the treatment if syphilis test is positive is explained completely, follow up etc and if the HIV test report is positive than the ICTC counsellor will explain and will do counselling(pre-test counselling and post-test counselling ) then will refer the client with all the relevant documents carrying with them to Nodal ART centre for further process of starting ART treatment.

ART CENTRES

ART Centres are divided into small units these areas under at medical colleges ART centres and at District Level Link ART Plus centres and FI ART centres. In ART centres those client reaches there who are found confirmed HIV Positive at ICTC centres/FI city centres and there their CD4 count is done after that ART Medicines started.

ART PLUS:-

Where registered patients alive on ART are 2000+ and this centre is opened in medical colleges and at the District level.

ART CENTRE:-

The ART Centre is opened where registered HIV+ve alive patients are more than 500+ than  ART Centre is opened in that district, in ART CD4 count is done after that medicines are started.

FI ART:-

This centre is opened where registered100+ HIV+ve patients are alive at Block in a District, and CD4 Is done in FI ART.

LAC PLUS:- 

This centre is Opened at the District level the centre should have more than 100 HIV positive alive. Medicines are made available to them at this centre. Then this centre is opened in that block.

LINK ART CENTRE:-

There should be 50 to 40 patients alive on ART.

Tab- TENOFOVIR DISPROXIL FUMARATE, 300mg, 245mg.

Tab- LAMIVUDINE, 300mg, 30mg, 150mg.

Tab- ZIDOVUDINE, 60mg, 300mg.

Tab- NEVIRAPINE, 200mg, and Syrup

Tab- ABACAVIR SULPHATE, 600MG.

Tab- DOLUTEGRAVIR, 50MG.

Tab- ISONIAZID, 300MG.

Tab- EFAVIRENZ, 600mg,

These medicines are given in combination of two or three medicines

ZLN: - ZIDOVUDINE + LAMIVUDINE+NEVIRAPINE

AL: - ABACAVIR SULPHATE + LAMIVUDINE.

ZL: - ZIDOVUDINE + LAMIVUDINE.

TLE: -   TENOFOVIR DISPROXIL FUMARATE+ LAMIVUDINE+ EFAVIRENZ

TL: - TENOFOVIR DISPROXIL FUMARATE + LAMIVUDINE.

DLT: - DOLUTEGRAVIR+LAMIVUDINE+TENOFOVIR DISPROXIL FUMARATE
